How to Become a Sole Trader - This resource introduces you to how to become a Sole-Trader or Independent Support Worker for self-managed or plan managed NDIS participants. Service Agreement - A Service Agreement defines the rights and responsibilities of the NDIS participant, and your rights and responsibilities as the Service Provider. A Service Agreement also describes how services and supports are to be arranged and delivered, thus protecting both the participant and the provider. This 5-page document provides information on your Quality and Safety Commitment to your participants, the responsibilities of your participant, payment requirements, cancellation policy, the process for ending or changing the agreement, and information regarding feedback and complaints.
Client Profile - Gather all the information that you need to set up your client’s records, including whether they are Plan or Self-Managed, and who to send your invoices to.
Consent & Privacy Agreement - In Australia, personal information is protected by strict privacy rules. To provide high quality and safe services, you may sometimes need to collect and use your client's personal information. For example, report writing, stakeholder meetings, and assisting with NDIS planning/budgeting/goals. To do this you will require your client’s consent.
Support Worker Progress Notes: Support workers are expected to maintain case notes every shift as part of their role. Progress or shift notes are documents created by support workers at the end of their shift and are essential part of a clients record/file. They are formal evidence of the delivery of services and protect the support worker and the participant.
